New Patient Form Helps Us Improve Our Communications With You
February 2, 2018Starting this month, we are introducing a new patient registration form. This form ensures:
- We have updated insurance and demographic information to appropriately bill for medical services
- Allows you to share your preference for receiving communications
- Lets us know with whom we can communicate about your care
In addition, by providing an email or cell phone, we will now be able to remind you about upcoming appointments by text or email, in addition to telephone calls.
Our new form is available online under Manage Your Care/Patient Forms. Or, you can fill it out at our office when you register.
