The Starling Physicians Advance Care Clinic was established in January 2024 under the leadership of Dr. Hoda Salam to support patients facing some of the most complex moments in their health journey. These individuals often live with multiple chronic conditions, frequent hospitalizations, and significant social or behavioral challenges. For them, this stage of life can feel overwhelming—and we believe they deserve more time, more attention, and more support than a traditional visit can offer.

Our clinic was built with that purpose at its core.

Today, our growing team of four dedicated providers delivers extended visits, thoughtful care planning, and deeply coordinated support across three Starling locations:

• 1260 Silas Deane Highway, Wethersfield
• 160 Hazard Avenue, Enfield
• 300 Kensington Avenue, New Britain

Our Most Vulnerable Patients (MVPs) represent the 15–20% of individuals who require the highest level of medical care. They are navigating complex illnesses, multiple medications, and frequent transitions between care settings.
