Chronic Care Management
An estimated 117 million adults have one or more chronic health conditions, and one in four adults has two or more chronic health conditions. Between doctor visits, taking the right medications at the right times, and following advice from different healthcare professionals, managing chronic conditions can be stressful.
At Starling, we believe there is tremendous value in taking care of our patients all of the time, not just at the point of care in the doctor’s office. Medicare shares this belief, which is why they have created the Chronic Care Management (CCM) program.
Managing your health care can be overwhelming, but it doesn’t have to be.
About Chronic Care Management (CCM)
The CCM program is designed to provide 24/7 care coordination services to Medicare beneficiaries with two or more chronic conditions. A chronic condition may include arthritis, asthma, diabetes, cancer, COPD, depression, dementia, hypertension, heart disease, and other conditions.
If you have two or more chronic conditions that are expected to last at least a year, you are eligible to enroll in the CCM program. As a member, you will be assigned a dedicated health professional who will check in with you regularly about your health for at least 20 minutes a month. This individual will reach beyond the office visit and engage with you about your care so you can make proper judgment calls about serious conditions and receive the support you need to manage your health.
As a member of CCM, your dedicated healthcare professional will work with you to create a comprehensive care plan to help you live a healthier life. The plan will include your health problems, your healthcare providers, medications, community services you have and need, your goals, and other information about your health. It also explains the care you need and how your care will be coordinated.
Your care coordinator will support you day and night, looking out for your best interests. Below are a few examples of the many services he/she may provide:

– Assist in locating resources to help with your medical needs
– Help you follow through on your doctor’s instructions
– Reconcile your medications, helping you avoid potentially dangerous drug interactions
– Ensure you know about and get access to the preventive care you need
– Locate resources for you that can help improve your health and lifestyle
– Provide expert assistance with setting and meeting your health goals
– Support you when you move from one healthcare setting to another
– Ensure you have 24/7 emergency access to a healthcare professional
– Coordinate your care between your pharmacy, specialists, testing centers, hospitals, and more.

How to Enroll
To participate in the CCM and get the connected care you need, talk to your Starling provider who can discuss the program more fully and ensure your eligibility. Your provider can help you enroll by completing a brief form.
